Thanks to your commitment to our mission, we have been able to address some of the challenges and we are making positive impact on the health and well-being of our community members. Here are some key achievements of  TGR community health center over the past year:

Increased Outreach Efforts:

We have expanded our outreach programs to reach a wider underserved population.

TGR health care staff and volunteers have conducted health education sessions and workshops on various topics, including preventive care and chronic disease management.

Enhanced Medical Services:

TGR Community Health Center have implemented medical programs to address specific health concerns prevalent in the community, such as diabetes management, prenatal care, and child wellness programs.

Expanded the range of services offered, including immunization.

Improved Access to Care:

Tender Grassroots has forged a Collaborative relationship with local schools and community organizations to promote health and wellness initiatives, such as school health screenings and health fairs.

TGR has served 9,873 patients over the past year and anticipate the number to be higher this year.
