Autism class 3 moth report 2023

S: the client was so cooperative in the session and he like to take occupational therapy sessions and he show that by laughing and hugging and the child is very distracted in the session he show that by play in everything and watch everything out side
O: the child was seen for 1-1 sessions for 45 minutes in OT room for tolerance and fine motor activity for tolerance the child do to put sticks in the hols and put the balls in the box with minimal assistance for fine motor activity the child play with slime and cut and rotate it and play with Lego he do it with minimal assistance. 
A: the child is 4 years old male diagnosed with mild cerebral palsy (CP) the child was very interested to play with balls because its an active game. Problem the child was very distracted in the session because the smallest of the place he need a minimal assistance 
Progress: the progress seen in the child in the tolerance activity he now can do a lot of activity in the session without distracted 
Benefit: the child will benefit form this activity that will strength his fine motor muscles and increase his tolerance 
P: the child will continues taking sessions in the OT room for 2 months for 1 hour per session for fine motor activity sensory stimulation and tolerance activity 
Theriapy  note 
S: the client was very cooperative in the session and she like to be motivated and show that by say “im smart” 
O: the child was seen for 1-1 sessions for 45 minutes in OT room for  pre-school skills the child do following line and drawing in the shapes with moderate assistance and he use an adaptive shapes to draw without get out from the line 
A: the child is 15 years old male diagnosed with cerebral palsy the child very interested in the session because he like to draw and use the pencils problems the child cant He can't hold the pen in the right way and extorts a lot 
Progress: the child can follow the line good and draw without get out from the line and become to know how to hold the pen in the right way 
Benefit: the child will be good in pre-school skills 
P: the child will continues taking sessions in the OT room for 2 months for 45 minutes for tolerance and pre-school skills and concentration.
Theriapy  note 
S: the child was cooperative in the session and she show that by do all the activity. Smile And enjoy the sessions she was afraid in the end off the session without reason and she show that by run away and hide behind the wall 
O: the child was seen for 1-1 session for 45 minutes in the OT room for tolerance, fine motor, pre school skills and concentration for tolerance the child do a puzzle and put the beads in the rope she do the activity with minimal assistance for fine motor the child do Tighten the screws and lego and she do the activity with supervision assistance for pre school skills the child do following line and coloring in the frame she do it with moderate assistance. 
A: the child is 23 years old female diagnosed with cri-du-chat syndrome the child show that she is interested in the activity for fine motor put the beads in the rope because make rosary beads at home problems the child afraid from everything and run away. She need a minimal and moderate assistance in general. 
Progress: no progress show at present 
Benefit: the child will work on her fine motor ,pre school skills and tolerance.
P: the child will continues taking sessions in the OT room for 2 months for 45 minutes for tolerance and pre-school skills and fine motor. 
Theriapy  note 
S: the client was so cooperative in the session and he show that by do all the activity and he show that he is very happy by say jokes and he say i like to play with the Dumbbells. 
O: the child was seen for 1-1 session for 45 minutes in the OT room for fine motor and cognitive activities for fine motor the child do a dumbbells with supervision assistance and Tighten the screws and lego game with minimal assistance and for cognitive activity the child do puzzle and remember the  photos on the cards with moderate assistance 
A: the child is 8 years old male diagnosed with cerebral palsy “cp” he like to play with dumbbells  because he want to build a muscle problem the child has a weakness in his fine motor and he cant do all the activities 
Progress: no progress show in the present 
Benefit: the child will work on his fine motor and memory skills and cognitive abilities 
P: the child will continues taking sessions in the OT room for 2 months for 45 minutes for cognitive and fine motor activities 
Theriapy  note 
S:the client was so cooperative in the session and he show that by done all the activity and he like to take the session by joking laughing and laughing enjoyed done the activity. 
A: the child was seen for 1-1 session for 45 minutes in the OT room for balance the trunk, fine motor, and cognitive activities for trunk balance the child set on chair without back and do a reaching activity put the Circular rings in the sticks in different way the client do it with minimal assistance for fine motor the child done an slime and put the balls in the tube using tweezers with physical moderate assistance for cognitive the child done shapes puzzle activity with minimal assistance. 
A: the child is 4 years old male diagnosed with mild cerebral palsy (CP) he was very cooperative helpful and do all the activity will problems the child distracted because theres a lot of noise in the room and  he show a lot of difficulty in balance the trunk at first. He need a minimal assistance in general 
Progress: no progress show 
Benefit: the child will work on fine motor skills trunk balance and cognitive activities 
P: the child will continues taking sessions in the OT room for 2 months for 45 minutes per session for fine motor activity sensory stimulation and tolerance activity 
Theriapy  note 
S: the child was uncooperative in the session and he show that by run away and refusing to do some activity. 
O: the child was seen for 1-1 session for 45 minutes in the OT room for following instructions activity sensory stimulation and cognitive activity for following instructions the child done take a funnel and put it in the other place he need a physical moderate assistance for sensory stimulation the child stimulated by water bubbles game independently  for cognitive he do a puzzle game with minimal assistance 
A: the child is 8 years old diagnosed with ADHD and behavioral issues problems the child was very uncooperative screaming run away he refused to do a lot of activities hets everyone but he is good in physical activities  and  fine motor activities he do all the activities with minimal assistance in general 
Progress: no progress show 
Benefit: the child will work on cognitive, following instruction and tolerance. 
P: the child will continues taking sessions in the OT room for 2 months for 45 minutes per session for fine motor activity sensory stimulation, tolerance activity and following instructions. 
 
Theriapy  note 
S: the client was cooperative in the session and he show that by done all the activities
